Exosomes & SVF

Regenerative aesthetics is a field that has developed rapidly in recent years and attracts a great deal of curiosity. Two terms heard often within it — “exosomes” and “stromal vascular fraction (SVF)” — may sound similar, but they are in fact quite different things.

What Are Exosomes?

Exosomes are natural particles of very small size (on the scale of a thousandth of a micron) that cells use to communicate with one another. An exosome secreted by one cell carries “messages” such as proteins, growth factors and genetic material to other cells, and can influence how those cells behave.

Exosome products used in aesthetics are generally obtained from stem cells produced in a laboratory (frequently derived from cord blood), or synthetically (biomimetic). They are therefore not taken from the person's own body but sourced externally. In skincare they can be applied to the skin after laser or microneedling, and in hair treatments to the scalp; the aim is to support the tissue repair process.

The scientific literature on aesthetic use of exosomes is still maturing. The data so far indicates that their role is strongest as an adjuvant immediately following a controlled tissue stimulus such as laser or microneedling; on their own, they are not a “miracle solution” that replaces other treatments. Because the source of the product, its manufacturing quality and its sterilisation standards can differ between products, working with reliable and transparent sources matters a great deal. The regulatory framework for exosome products worldwide is also still developing.

What Is Stromal Vascular Fraction (SVF)?

Stromal vascular fraction (SVF) is a mixed cell population obtained from the person's own fat tissue (autologous) through a small liposuction procedure. It contains stem cells derived from fat tissue, vessel wall cells (endothelial cells, pericytes) and various growth factors.

The most important characteristic of SVF is that it is obtained from the person's own tissue; unlike exosomes it is not an external product but a processed component of the patient's own fat. This cell population is thought to support the formation of new vessels (angiogenesis), reduce inflammation and contribute to tissue repair.

In aesthetic and reconstructive surgery, SVF most often comes up in support of fat grafting: when fat tissue is enriched with SVF before injection, it is thought to contribute to the survival and longevity of the transferred fat. In this respect SVF can be considered a supportive option for fat grafting used in breast or body contouring. In scar areas with advanced deformation (such as acne or burn scars), SVF is a good option for improving tissue quality.

The literature on SVF is likewise still actively developing; the method of preparation (mechanical or enzymatic processing) can affect cell quality and results. Whether the method is suitable for a given person, which procedure it would be combined with, and the limits of the expected contribution should be discussed in detail during the examination.
