1. Data Controller

Under the KVKK, your personal data is processed by Op. Dr. Işıl Akgün Demir as data controller, within the scope described below.

2. Personal Data Processed

Only the following data is processed through the contact form on this site:

• Identity data: first and last name
• Contact data: phone number
• Request data: the procedure you indicate an interest in, and anything you share in the message field

Please do not share detailed health information through this form. Any assessment of your medical condition is made only during an examination.

3. Purposes of Processing

Your personal data is processed for the following purposes:

• Receiving, recording and responding to appointment and consultation requests
• Evaluating your request and contacting you
• Fulfilling obligations arising from applicable legislation

4. Legal Grounds

Your personal data is processed under Article 5/2 of the KVKK on the grounds that “processing is necessary for the establishment, exercise or protection of a right” and that “processing is necessary for the legitimate interests of the data controller, provided that it does not harm the fundamental rights and freedoms of the data subject”.

For any statement that may constitute health data, processing relies solely on the explicit consent you give under Article 6 of the KVKK. Pressing the “I Approve” button before sending the form serves to obtain that explicit consent.

5. Transfers

Your data may be transferred, limited to the purposes above and only to the extent necessary, to suppliers providing communication and IT infrastructure services, and to legally authorised public institutions upon request.

The contact form prepares your message to be sent via the WhatsApp application. Once sent, your message is therefore also subject to that platform's own privacy policy and terms of use.

6. Method of Collection

Your personal data is collected electronically, entirely on the basis of your own declaration, through the contact form on this website and via WhatsApp. No marketing or profiling cookies are used on this site.

7. Retention

Your personal data is erased, destroyed or anonymised once it has been kept for as long as necessary for the purpose of processing and for the limitation periods set out in the applicable legislation.

8. Your Rights under Article 11 of the KVKK

As a data subject you have the right to:

• Learn whether your personal data is being processed
• Request information if it has been processed
• Learn the purpose of processing and whether the data is used in accordance with that purpose
• Know the third parties to whom the data is transferred, in Turkey or abroad
• Request correction if the data is incomplete or inaccurate
• Request erasure or destruction of the data
• Request that correction, erasure and destruction be notified to third parties to whom the data was transferred
• Object to a result arising against you from analysis exclusively by automated systems
• Claim compensation if you suffer damage due to unlawful processing

9. Applications

You may submit requests relating to your rights to the data controller using the contact details above. Requests are concluded as soon as possible and in any case within thirty days at the latest.
